Patient Appointments

Except in the case of an emergency, we prefer to see patients by appointment only. This allows us to schedule an adequate amount of time for your visit.

New patients should view What to Expect for information about your first appointment.

If you are an existing patient who has moved or changed insurance, please download the registration form, complete it with your new information and bring it with you on your next visit.

Billing and Insurance Questions

Please contact our Billing office (602) 973-0455 with any financial questions. They will be happy to answer anything related to the financing of your care.

For women that qualify, you can apply for AHCCCS right here in our office. There are also affordable payment plans set up to meet the individual financial needs of women that do not have any insurance benefits. You will be referred to our Patient Account Representative to discuss your financial planning.
