To accommodate family schedules, we have early morning hours from 7:30 AM and late hours until 7:00 PM.
Patient Appointments and Forms
Except in the case of an emergency, we prefer to see patients by appointment only. This allows us to schedule an adequate amount of time for your visit. If you have insurance, please bring your card and information with you. We will make a copy of your card for our records and return it to you before your visit.
New patients should download and complete the registration form and the HIPAA form and bring them with you to your first appointment.
If you are an existing patient who has moved or changed insurance, please download the registration form and complete it with your new information and bring it with you on your next visit.
Referrals/Prior Authorization
While it is the responsibility of the insured member to obtain a referral prior to an appointment, our office will assist in any way possible to see to it that proper referrals and authorizations are received.
Billing Office
Please contact our billing office 602.973.0455 with any financial questions. They will be happy to answer anything related to the financing of your care. For women that qualify, application for AHCCCS assistance is possible at the office. There are also affordable payment plans that are set up to meet the individual financial needs of women that do not have any insurance benefits. You will be referred to our patient account representative to discuss your financial planning.
